HIPAA Notice
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Effective Date: January 1, 2026
1. SCOPE This Notice applies to the independent, physician-owned medical practices that deliver care to patients through the Averion Health LLC platform. Averion Health LLC itself is not a HIPAA-covered entity; however, Averion Health LLC acts as a Business Associate of the covered practices and is contractually bound to protect PHI in accordance with HIPAA.
2. OUR COMMITMENT We are required by law to maintain the privacy of your protected health information ("PHI"), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.
3. HOW WE MAY USE AND DISCLOSE PHI Treatment — to provide, coordinate, and manage your healthcare, including consultations, prescriptions, and lab work. Payment — to bill and collect payment for services, which may include contacting payment processors. Healthcare Operations — quality assessment, provider training, compliance, and operational planning. As Required by Law — public-health reporting, judicial orders, or other legally mandated disclosures. Business Associates — third parties who perform services on our behalf under HIPAA-compliant agreements.
4. YOUR RIGHTS Right to Access — request a copy of your PHI. Right to Amend — request correction of PHI you believe is inaccurate. Right to an Accounting — request a list of certain disclosures we have made. Right to Request Restrictions — request limits on how PHI is used or disclosed. Right to Confidential Communications — ask us to communicate with you in a specific way. Right to a Paper Copy — of this Notice, at any time.
5. USES REQUIRING YOUR AUTHORIZATION Most uses or disclosures of psychotherapy notes, marketing communications, and sale of PHI require your written authorization. You may revoke authorization in writing at any time.
6. COMPLAINTS If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
7. CHANGES TO THIS NOTICE We reserve the right to change this Notice and to make the revised version effective for PHI we already have as well as any we receive in the future. The current Notice is always posted on our website.
